The Case Manager provides comprehensive community-based treatment for individuals with severe mental illness who are involved with the legal/criminal justice system. This role involves aggressive outreach within the consumer's community to assist them in establishing more productive lives.
Responsibilities
- Provide case management activities through an aggressive Forensic Assertive Community Treatment Team outreach approach
- Administer interventions for individuals diagnosed with a mental illness involved in the criminal justice system
- Assist individuals with establishing more productive lives
Requirements
- Bachelor's degree in social work, psychology, counseling or other related field OR a bachelor's degree in a non-clinical field plus two years' experience
- Valid Pennsylvania Driver's License
Qualifications
- Bachelor's degree in social work, psychology, counseling or other related field, or a bachelor's degree in a non-clinical field plus two years' experience. Master's preferred.
