Case Manager- Forensic Assertive Community Treatment Team

About This Role

The Case Manager provides comprehensive community-based treatment for individuals with severe mental illness who are involved with the legal/criminal justice system. This role involves aggressive outreach within the consumer's community to assist them in establishing more productive lives.

Responsibilities

• Provide case management activities through an aggressive Forensic Assertive Community Treatment Team outreach approach
• Administer interventions for individuals diagnosed with a mental illness involved in the criminal justice system
• Assist individuals with establishing more productive lives

Requirements

• Bachelor's degree in social work, psychology, counseling or other related field OR a bachelor's degree in a non-clinical field plus two years' experience
• Valid Pennsylvania Driver's License

Qualifications

• Bachelor's degree in social work, psychology, counseling or other related field, or a bachelor's degree in a non-clinical field plus two years' experience. Master's preferred.
