Care Manager, Health Management - Registered Dietitian (RD)

About This Role

The Care Manager, Health Management supports health management activities within care management, coordinating integrated member care across various health needs. This role ensures member progress toward desired outcomes, contributing to quality and cost-effective care.

Responsibilities

• Provide care management or refer members to a higher level of care based on clinical assessment and reported health concerns
• Identify member needs, close health care gaps, develop action plans, and prioritize goals
• Educate members and families on managing chronic health conditions, preventing symptom progression, and adopting healthy lifestyle behaviors
• Provide general member education for self-management goals, disease management, or acute conditions
• Assess for barriers to care and assist members in addressing concerns
• Advocate for members to guide them through the health care system for transition planning and longitudinal care
• Reinforce medication adherence and education, monitoring member reactions to medications and treatments
• Engage member, family, and caregivers telephonically to establish and continually assess well-coordinated action plans
• Perform ongoing monitoring of care plans to evaluate effectiveness, document interventions, and suggest changes
• Maintain an ongoing member caseload for regular outreach and management
• Use motivational interviewing and Molina clinical guideposts to educate, support, and motivate change during member contacts
• Collaborate with registered nurse care managers/leaders as needed

Requirements

• 2+ years experience in health care
• 1+ year experience in direct patient care, managed care, care management, or behavioral health setting
• Licensed as a Registered Dietician (RD)
• Knowledge of community resources
• Proficiency in Microsoft Office suite/applicable software programs

Qualifications

• At least 2 years experience in health care, including at least 1 year of experience in direct patient care, and/or managed care, care management, or behavioral health setting

Nice to Have

• Certified Case Manager (CCM)
