This role is responsible for telephonically performing case management for members with behavioral health and substance abuse needs, using clinical judgment and expertise in specific clinical areas. The Behavioral Health Case Manager II will assess individual needs, develop care plans, monitor their effectiveness, and coordinate with internal and external resources to ensure appropriate care.
Responsibilities
- Respond to more complex cases and account-specific requests
- Use appropriate screening criteria knowledge and clinical judgment to assess member needs
- Conduct assessments to identify individual needs and develop specific care plans
- Monitor and evaluate effectiveness of care plans and modify them as needed
- Support member access to appropriate quality and cost-effective care
- Coordinate with internal and external resources to meet identified needs of members and collaborate with providers
- Serve as a resource to other Behavioral Health Case Managers
- Participate in cross-functional team projects and initiatives
Requirements
- Master's degree in social work counseling or related behavioral health field, or a nursing degree
- 3+ years of clinical experience in social work counseling with complex psychiatric and substance abuse treatment
- Current active unrestricted license such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, or Clinical Psychologist
- Previous experience in case management and telephonic/in-person coaching for members with complex psychiatric/substance abuse/medical disorders
- Managed care experience
Qualifications
- MA/MS in social work counseling or a related behavioral health field or a degree in nursing
- Minimum of 3 years of clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment; or any combination of education and experience which would provide an equivalent background. Previous experience in case management and telephonic and/or in person coaching with members with a broad range of complex psychiatric/substance abuse and/or medical disorders. Managed care experience required.
