About This Role

This role involves telephonic case management for members with behavioral health, eating disorder, and substance use disorder needs. Key responsibilities include conducting assessments, developing care plans, and coordinating resources to ensure appropriate and cost-effective care.

Responsibilities

  • Manage a caseload of adult and adolescent members with behavioral health, eating disorder, and substance use disorder needs
  • Make daily outbound calls to enroll and engage members
  • Take inbound calls from members to assist with needs/resources
  • Use screening criteria and clinical judgment to assess member needs
  • Conduct assessments to identify individual needs and develop care plans
  • Monitor and evaluate effectiveness of care plans and modify as needed
  • Support member access to appropriate quality and cost-effective care
  • Coordinate with internal and external resources to meet identified needs and collaborate with providers

Requirements

  • MA/MS in social work, counseling, or a related behavioral health field or a degree in nursing
  • Minimum of 3 years clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment
  • Current active unrestricted license such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, or Clinical Psychologist to practice as a health professional within the scope of licensure in applicable states or territory of the United States

Qualifications

  • MA/MS in social work, counseling, or a related behavioral health field or a degree in nursing
  • Minimum of 3 years clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment

Nice to Have

  • Experience in case management and telephonic and/or in person coaching with members with a broad range of complex psychiatric/substance abuse and/or medical disorders
