Baylor Weekend Physical Therapist, Home Health

About This Role

As a Home Health Physical Therapist, you will provide personalized skilled physical therapy treatment to patients in their homes, helping them regain strength, mobility, and independence. You will plan and administer treatment, evaluate patient progress, and collaborate with physicians and clinical team members to achieve optimal results.

Responsibilities

• Plan and administer prescribed skilled physical therapy treatment and training for patients to attain their highest level of physical function.
• Test and screen patients' physical strengths to assist in evaluating their function and develop or pursue treatment programs.
• Develop and implement conditioning/rehabilitation programs consistent with the physician's Plan of Treatment and overall goals.
• Confer with physicians and clinical team members to develop, implement, and revise therapy treatment programs.
• Provide Physical Therapy Assistants and Home Health Aide staff with written instructions/care plans and supervise/evaluate their performance and documentation.
• Accurately, promptly, and thoroughly document patients' care observations, interventions, and evaluations.
• Report patient's progress to the patient's physician, Clinical Manager, staff, and family.

Requirements

• Degree from an accredited Physical Therapy Program (approved by the APTA)
• Current and unrestricted Physical Therapy license
• Current CPR certification

Qualifications

• Degree from an accredited Physical Therapy Program (approved by the APTA)
• Minimum of one year physical therapy experience preferred

Nice to Have

• Minimum of one year physical therapy experience
