Wraparound Facilitator

About This Role

The Wraparound Facilitator provides assessments to identify strengths and needs for treatment/service planning, monitors service delivery, and helps build support networks for children and families. This role coordinates and creates services to fit unique family needs, building upon their strengths to create lifelong skills and relationships.

Responsibilities

• Provide assessments to identify strengths and needs for treatment/service planning
• Monitor service delivery, goals, and problem-solving
• Help build a support network for families
• Coordinate and create services and supports to fit unique needs of children and families
• Build upon family strengths to create lifelong skills and relationships
• Perform Service Coordination duties
• Engage families and develop relationships
• Conduct assessments for treatment/service planning
• Develop service plans
• Evaluate program effectiveness
• Serve as a resource expert
• Act as a convener/facilitator for family meetings
• Provide linkage for supports and resources
• Demonstrate cultural competence in service delivery
• Advocate for children and families
• Ensure documentation and compliance with regulations

Requirements

• State Police/FBI per state regulations clearance
• Child Protection clearance
• Valid vehicle insurance
• 2 years relevant experience with youth and families in social services

Qualifications

• Bachelor's degree in Human Services or related field
• 2 years of relevant experience with youth and families in social services.

Nice to Have

• Master's degree in Human Services or related field
