About This Role

Provide responsible wellness services as a Licensed Practical Nurse (LPN) within an assisted living community, focusing on resident assessments, service plan development, medication administration, and incident documentation.

Responsibilities

  • Complete assessments to determine and provide appropriate level of care
  • Complete Service Plans within appropriate timeframes, including auditing and modifications for changes in resident condition
  • Perform initial or ongoing screenings to determine appropriate levels of care and physical/cognitive needs
  • Meet with residents, responsible parties, and the AL Manager for scheduled Service Plan meetings
  • Provide first aid to residents and request physician orders for ongoing treatment
  • Participate in identifying needs for, and acquiring, third-party services like Hospice, Home Health, PT, OT, ST, or SN
  • Complete documentation in the resident record for emergent situations and incidents
  • Administer medication, treatments, and biologicals to residents under physician’s orders
  • Monitor medication cabinets for discontinued or expired medications and handle disposition
  • Ensure caregivers confirm completed tasks in EMR daily and progress notes are comprehensive
  • Provide support, assistance, advice, and guidance to unlicensed staff in the assisted living setting
  • Correctly query, modify, and create information in the resident’s electronic record
  • Ensure resident medical or health problems are supported as addressed in the Service Plan
  • Lift, move, and transport residents when necessary, using proper body mechanics or lifting devices

Requirements

  • Thorough knowledge of professional nursing practices/theories, techniques and tactics
  • Excellent oral, written, presentation and interpersonal skills
  • Computer proficiency (Microsoft Word/Excel, Outlook, Internet)
  • Organized, able to multi-task, think creatively and adapt to different situations
  • Ability to react successfully and calmly to situations with residents, guests, or staff
  • Ability to write routine reports and correspondence
  • Maintain competency in the position and knowledge of trade/industry advances and new technologies
  • Ability to effectively communicate in English with all levels of staff, residents, and guests

Qualifications

  • Successful completion of AZ State Board of Nursing approved licensed practical nurse program. Preferably a two-year degree from an accredited college or university in nursing, public health, or related field.
  • Preferably three to five years of progressive responsibility in a health care environment
