About This Role

As a Licensed Practical Nurse (LPN) at Charlestown Place, you will provide direct care to residents in a skilled nursing community. This role involves monitoring patient conditions, administering medications, and collaborating with an interdisciplinary team to develop and implement care plans.

Responsibilities

• Provide direct care to residents by monitoring their condition
• Administer medications and perform necessary tasks such as taking vital signs and documenting care
• Collaborate with the interdisciplinary team to develop and implement care plans
• Ensure that all care is compliant with regulatory standards
• Assist with staff training and participate in quality assurance activities
• Maintain clear communication with residents, families, and medical staff about changes in care

Requirements

• Must be 18 years of age or older
• Active LPN license in the state of Indiana

Qualifications

• At least one-year of related experience is preferred (but not required)
