About This Role

Provide physical therapy services to patients in their homes, helping them regain health and mobility by assessing their condition, developing treatment plans, and offering patient education.

Responsibilities

Provide PT services to help patients regain their health and mobility
Assess patients' mobility, strength and function at their homes
Develop treatment plans
Maintain patient records
Provide patient education
Help patients develop strength
Help patients reduce pain
Help patients improve function

Requirements

At least 1 year of recent experience in a home health setting
Degree in relevant field
BLS Certification (AHA)
Current state/compact license
