Travel Occupational Therapist

About This Role

Provide occupational therapy services in a skilled nursing facility/long-term care setting, assessing patients' functional capabilities and developing individualized treatment plans.

Responsibilities

• Assess referred patients requiring rehabilitation services, including medical condition, functional capabilities, limitations, restrictions, and rehabilitation potential.
• Establish and administer a treatment program with specific, achievable goals under physician direction.
• Document and record patient conditions and educate patients and families on appropriate occupational therapy methods.
• Provide skilled occupational therapy services/interventions in accordance with physician orders.
• Assess patient needs, plan for, evaluate, and modify care to meet goals of occupational therapy interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.

Requirements

• Graduated from an accredited school
• Current state license as an Occupational Therapist / OT (or proactively in the process of applying)
• Current CPR certification

• Graduated from an accredited school
• Experienced or newly graduated Occupational Therapist
