Join a skilled nursing facility as a travel Occupational Therapist in South Bend, Indiana, providing direct patient care, assessing medical conditions, and establishing rehabilitation treatment programs.

Responsibilities

• Assess referred patients requiring rehabilitation services
• Provide direct patient care to assess medical condition, functional capabilities, limitations, restrictions and potential for rehabilitation
• Establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician
• Set realistic and achievable goals for patients
• Document and record the patient's condition
• Educate patients and families in an appropriate occupational therapy method
• Provide skilled occupational therapy services / interventions in accordance with physician orders
• Assess patient needs, plan for, evaluate and modify care to meet goals of occupational therapy interventions
• Collaborate with all disciplines to plan and evaluate team goals for each patient

Requirements

• Must have graduated from an accredited school
• Current state license as an Occupational Therapist / OT or proactively in the process of the application process for current state licensure as an Occupational Therapist / OT
• Current CPR certification

Qualifications

• Graduated from an accredited school
• Experienced or newly graduated Occupational Therapist
