This role involves managing and overseeing the Case Management and Social Services Departments while also performing all functions of Case Management to facilitate effective and efficient patient care during hospital stays.
Responsibilities
- Facilitate effective and efficient patient care during the hospital stay
- Conduct face-to-face visits with patients and/or family members for comprehensive assessment of discharge needs
- Provide clinical information to third-party payers to support patient admission and continued hospital stay
- Communicate actively with physicians regarding length of stay, admission/treatment criteria, and cost-effective service delivery
- Collaborate on the formulation of appropriate and realistic patient-centered discharge plans
- Arrange necessary follow-up services including durable medical equipment, home health, and/or placement
- Act as a liaison between patients, families, visitors, physicians, SDMC staff, and community service providers
- Participate in and/or facilitate interdisciplinary care conferences
- Supervise and ensure staffing in the Case Management and Social Services Departments
Requirements
- Current Registered Nurse License (Texas License must be obtained within 90 days if Compact License)
- Basic Cardiac Life Support (within 90 days of employment)
- Bachelors Degree or Masters Degree
- 5+ years of Case Management experience in an acute care setting
- 2+ years of supervisory or management experience

- 5+ years of Case Management experience in an acute care setting, plus 2+ years of supervisory or management experience
