Social Service Designee

About This Role

This role assists residents and families in adjusting to placement, illness, and their care plan within a post-acute facility. The designee helps residents achieve their highest practicable level of functioning while adhering to regulations and policies.

Responsibilities

• Assist each resident and family in adjusting to placement, illness, and the plan of care
• Help residents attain the highest practicable level of functioning in accordance with current federal and state regulations, and policies and procedures
• Assist residents and families with wishes regarding treatment and help with their transition and adjustment to the facility
• Participate in resident and family council meetings as requested
• Support Admissions and assist with discharge planning services

Requirements

• High school diploma or equivalent
• Ability to work with physically and mentally ill, disabled, and emotionally upset elderly individuals
• Ability to organize, prioritize, and meet deadlines
• Ability to maintain confidentiality and be discreet
• Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care

Qualifications

• High school diploma or equivalent

Nice to Have

• Experience with geriatric population
• AA or Bachelor's degree in a related field
• Creative problem-solving skills
• Strong communication skills
• Sufficient computer skills to manage records and documentation
