About This Role

The Social Service Aide/Designee assists the Social Service Director in planning and organizing the Social Service Department, ensuring compliance with policies, standards, and regulations to meet individual resident needs and facility goals.

Responsibilities

  • Assist the Social Service Director (SSD) to plan, develop, and organize the Social Service Department
  • Ensure department operations comply with current Alden policies, Federal, State, and local standards
  • Welcome new admission residents, provide support, and initiate trusting/therapeutic relationships
  • Assess and document residents' feelings about long-term care placement, emotional, mental, and physical limitations
  • Become knowledgeable of each individual resident’s background, cultural history, and medical care needs
  • Document interactions with residents and families in assessments, care plans, and social services notes
  • Prepare a plan of care for treatment with the Interdisciplinary Team (IDT) based on Comprehensive Assessment
  • Imitate, facilitate, and/or participate in the written discharge plan
  • Make supportive visits to residents, writing progress notes at least quarterly or when notable changes occur
  • Provide supportive counseling and behavioral intervention/programming to residents
  • Act as resident advocate and liaison between residents, families, facility, and community agencies
  • Maintain adequate record system for obtaining, recording, and filing social service information
  • Conduct, oversee, and complete initial and all on-going assessments, MDS, CAAs, and care planning initiatives
  • Provide scheduled and/or spontaneous education for families, residents, and staff.

Requirements

  • High School Diploma
  • Two years of experience in social service/case management in healthcare
  • Current knowledge of Federal and State Long-Term Care (LTC) Social Service regulations
  • Ability to read, write, and speak English in an understandable manner
  • Ability to make independent decisions in the best interest of residents, staff, and families
  • Ability to deal tactfully with residents, family members, visitors, and personnel
  • Ability to produce concise, meaningful, clinical written reports and assessments
  • Ability to articulate the social/psychological needs of residents
  • Ability to handle confidential data with professional discretion

Qualifications

  • High School Diploma required, Bachelor’s Degree in a Human Services field preferred
  • Two years of experience in working in the social service/case management realm of the healthcare field

Nice to Have

  • Bachelor’s Degree in the Human Services field (e.g., Social Work, Psychology, Counseling, Sociology)
  • Experience and qualified marketing skills to express Alden’s Social Services Program
  • Sincere desire to work with populations requiring long-term or short-term care
