Shelter Case Manager

About This Role

Provide essential case management and counseling services to homeless adults, focusing on securing financial entitlements, housing, and promoting wellness and independent living skills.

Responsibilities

• Provide case management and counseling services by conducting intake, psychosocial and housing assessments of clients
• Identify and obtain financial entitlements, housing, and other concrete services (Health Insurance, Food Stamps, SSI etc.) for clients
• Collaborate with each client to prepare and monitor an Independent Living Plan, identifying long and short-term objectives
• Identify and make referrals for community-based treatment and support services
• Counsel clients through individualized and group sessions on issues such as wellness self-management, maintaining sobriety, complying with medication, pursuing employment, and independent living skills
• Collaborate with on-site psychiatric provider/nursing/primary care to ensure each client has an aftercare plan
• Maintain accurate and up-to-date written client documentation as required by regulations and guidelines
• Input client information and updates into the database system

Requirements

• Bachelor’s Degree in Social Work or related field
• Proficiency in computer skills
• Excellent oral, writing and listening skills

Qualifications

• Bachelor’s Degree in Social Work or related field
• 1-2 years of experience working with homeless mentally ill adults or other special needs population preferred

Nice to Have

• 1-2 years of experience working with homeless mentally ill adults or other special needs population
• Bilingual or multilingual
