RN Care Manager Home Care

About This Role

Serve as the primary RN Care Manager for home care clients, developing and implementing individualized care plans, and coordinating care among interdisciplinary team members. Provide high-quality, compassionate care to older adults in their homes.

Responsibilities

• Collaborate with interdisciplinary team members to develop and implement individualized care plans
• Provide constant feedback to clients, client representatives, caregivers, and nurses
• Work closely with the office team to ensure excellent communication and problem-solving
• Serve as the primary care manager, coordinating care to clients among caregivers, RNs, therapists, and outside representatives

Requirements

• Active California RN License
• Strong computer skills
• Ability to multitask, drive, take action
• Strong organizational & communication skills
• Pass Live Scan Fingerprint Clearance
• Valid driver's license and proof of insurance
• Cell phone with Internet Access
• Proof of Negative TB results

Qualifications

• Master in Nursing, preferred
