This role involves conducting medical and behavioral health assessments, managing medication advocacy, and providing crisis intervention for older New Yorkers in a residential setting. The aim is to prevent recidivism of hospitalization or homelessness and improve quality of life.
Responsibilities
- Conduct medical health assessments, behavioral health pre and post evaluations, care stabilization, and referral services to clients in need of follow up appointments
- Work with the Residential Coordinator to update and maintain medication advocacy, ensuring all medication changes are documented and followed
- Ensure clients receive an integrated, multidisciplinary assessment and immediate stabilization plan using a client-centered approach
- Perform with demonstrated ability and sensitivity providing services to highly vulnerable individuals with history of homelessness, substance affected, and/or serious mental illness
- Refer individuals to immediate/urgent care and establish community connections with primary care, mental health services and substance use disorder care
- Provide preliminary health diagnosis, care, and treatment plans to clients
- Provide crisis intervention assistance when needed
- Document each client interaction, health data, and notes in the AWARDS system
- Coordinate and follow-up on internal/external doctors’ appointments, hospital and rehabilitation admissions, including healthcare assessments
- Administer medication as prescribed when needed, including ointments, injections, ophthalmic
Requirements
- Sensitivity and empathy for all residents
- Current Registered Nurse Licensure in NY State
- Psychiatric Mental Health Nurse Certification
- Commitment to providing care services for individuals experiencing homelessness, mental health disorders, and/or substance use
- Demonstrated experience working with diverse educational, lifestyle, ethnic and cultural origins
- Excellent interpersonal skills and ability to work as both part of a team and independently
- Strong written and verbal communication skills
- A proactive person-centered approach
- Ability to triage emergencies when needed
- Flexibility in schedule required
- Commitment to Encore’s mission and to uplifting the voices of people with lived experiences of homelessness and mental health disorders
- Desire to perform the essential functions of this job
- Ability to handle emergency situations
Nice to Have
- Active Medicare/Medicaid Registration
- Proven experience with seniors who are formerly homeless and/or have mental health diagnoses
- Expert written, oral, and interpersonal communication skills
- Experience working both independently and in a team-oriented and collaborative environment
- Competency in using relevant patient care data base systems, preferably AWARDS
- Excellent organization skills
- Works well under supervision
- Works well and takes initiative in a fast paced, high intensity and ever evolving environment
