Join our Clinical Homecare Nursing team to provide individualized quality nursing care to patients requiring Parenteral Nutrition and other IV therapies in their homes. This role involves administering prescribed IV therapies, providing essential training and support to patients and carers, and participating in the on-call rota to ensure seamless service delivery.
Responsibilities
- Administer prescribed IV therapies and associated care and support to patients who are unable to be trained to independence
- Provide training, education and support to patients/carers with regards to the equipment and infusion devices required for infusion/administration of therapies to independence
- Provide training, education and support to patients/carers in the connection and disconnection of PN and IV therapies to independence
- Provide training, education and support patients/carers in the care and management of their CVAD
- Participate on the on-call rota, make autonomous decisions to resolve out of hours queries and escalate to the Senior on-call if required
- Work with the Practice Development Nurse and Regional Lead Nurse in discharge management including pre-discharge assessment visits and home assessments
- Support the Installation of new TransCare patients and complete all relevant documentation
Requirements
- Intensive Care / High dependency / General surgery/ nursing care experience at a level equal to top of Band 5
- Current clinical experience minimum of last 2 years
- Competent in the management and delivery of PN and other IV Therapies
- Competent in the care and management of CVADs
- Ability to perform blood samples from CVADs
- Registered General Nurse with current NMC qualifications
- Excellent communication skills
- Time management skills
- Ability to make correct clinical decisions in a clinical emergency situation
- Flexibility to travel to support Homecare patients
Qualifications
- Registered General Nurse with current NMC qualifications
- Minimum 2 years of current clinical experience at a level equal to top of Band 5
