The Recovery Support Coordinator is a vital member of the care team at Groups, providing direct support to members in their substance use disorder recovery journey. This role involves coordinating care across clinical, social, and community settings, assisting members with Health-Related Social Needs, and promoting overall member wellness.
Responsibilities
- Collaborate with a multidisciplinary care team to ensure high-quality, member-centered care.
- Build trusted relationships to assist members in achieving recovery goals through empathy and motivational interviewing.
- Assist members in identifying personal and treatment-related goals, providing individual coaching and coping skill support.
- Support unengaged members to reengage in treatment services through intensive outreach.
- Link members to healthcare, behavioral health, social service, and community-based resources and follow up to confirm needs are met.
- Assist members in navigating practical systems such as insurance, pharmacy, housing, and transportation.
- Maintain accurate, timely, and compliant documentation in the electronic health record (EHR).
- Support daily operations, including group facilitation logistics, scheduling assistance, and office maintenance.
- Assist clinicians, providers, and members with treatment-related tasks, including prescription management and appointment scheduling.
Requirements
- High school diploma
- Reliable internet access (50M download / 10M upload) and strong WiFi if working remote
- Ability to meet state-specific background screening and pre-employment requirements
- Valid driver’s license and access to a vehicle (if role includes community travel)
Qualifications
- High school diploma required; Associate’s degree in Social Services, Human Services, Psychology, or related field preferred.
- Minimum of two (2) years’ experience in case management, care coordination, peer support, or a related field preferred; and experience working with individuals with substance use or mental health challenges preferred. Experience using Electronic Health Records (EHR).
