PRN/Per-Visit Nurse Practitioner - Health Risk Assessments

About This Role

This Nurse Practitioner role involves performing annual in-home or telehealth health risk assessments to identify active and chronic conditions, ensuring patient-centered care for individuals with complex healthcare needs.

Responsibilities

• Deliver annual risk assessment in a residential setting or via telehealth, within the scope of practice for a Nurse Practitioner
• Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions, as well as determine all physical, mental, and social needs present at the time of the visit
• Take history, examine, and determine diagnoses
• Provide written documentation of patient visit, per NCQA standards
• Take patient vital signs, as necessary
• Place case management referrals and communicate with PCP as necessary
• Communicate with patients, caregivers, agency nurses, other providers and vendors as necessary to assure proper diagnosis
• Perform all clinical duties while observing OSHA Universal Precautions
• Maintain patient confidentiality
• Attend required meetings and in-services and participate in committees, as requested
• Participate in professional development activities and maintains professional licenses and affiliations

Requirements

• Active/unrestricted nurse practitioner license to practice in coverage states
• Board certification in one of the following: American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP) or National Commission on Certification of Physician Assistants (NCCPA)
• Active BLS Certification
• Current enrollment in Medicare/Medicaid
• Must maintain a valid driver’s license and good driving record
• Outstanding EHR skills
• Reside within Bergen County, NJ

• Skill in teamwork and maintaining effective working relationships with patients, medical staff, and the public
