About This Role

Provide primary medical treatment and personal care services to patients, primarily veterans, within a Community Based Outpatient Clinic. This role involves diagnosing, treating, and managing patient care plans in collaboration with a PACT team.

Responsibilities

  • Actively diagnose and treat veterans under the direction of a supervising physician
  • Examine patients for symptoms of organic or congenital disorders
  • Develop and implement patient management plans and assist in continuity of care
  • Order and perform diagnostic tests and interpret results for deviations from normal
  • Counsel patients on prescription medications and provide routine health maintenance
  • Evaluate patient records from outside medical providers for co-managed care
  • Complete clinical reminders at the time of each patient visit
  • Document medical records within 24 hours of a patient encounter
  • Comply with VA guidelines for patient requests, alerts, consults, orders, and diagnostic studies
  • Comply with the VA formulary process and consult protocols
  • Comply with VA guidelines for timely clinical documentation, including encounter completion by close of business
  • Comply with all VA and company training requirements
  • Fulfill compliance requirements for OIG, JC, EOC oversight, and lab compliance
  • Provide excellent customer service to veterans and colleagues
  • Participate in clinic outreach events to support enrollment initiatives
  • Support patient enrollment and retention through scheduling and follow-up guidance
  • Embrace and support new clinical or operational initiatives

Requirements

  • Master of Science in Nursing (MSN) required
  • Minimum one-year clinical experience as a CRNP in primary or ambulatory care setting within the last 4 years
  • Credentialed and in good standing through the Veterans Health Administration (VA)
  • Valid, unrestricted Nurse Practitioner license in the state of desired employment or any U.S. state/territory
  • Current BLS certification
  • Proficiency in written and spoken English

Qualifications

  • Graduate of an accredited school of nursing and an accredited program for nurse practitioner (MSN required), including preceptorship
  • Minimum one-year clinical experience (three years preferred) as a CRNP within the last 4 years in a related primary care or ambulatory care setting; government healthcare environment preferred.

Nice to Have

  • Three years clinical experience as a CRNP
  • Experience in a government healthcare environment (preferred)
  • ACLS certification
