About This Role

This Physical Therapist role provides age-appropriate services for hospice patients, focusing on comprehensive assessment, direct treatment, and developing home exercise programs. The therapist will also instruct patients and families, participate in the Interdisciplinary Group, and document services according to hospice standards.

Responsibilities

  • Initiate physical therapy evaluation according to physician orders and Plan of Care for symptom control or to maintain activities of daily living and basic functional skills.
  • Document each patient visit in the clinical record and report patient's reaction to treatment, changes in status, comfort level, and follow-up to physician and Administrator.
  • Participate in the Interdisciplinary Group and provide applicable input for the patient's Plan of Care.
  • Instruct and inform members of the Interdisciplinary group, patient, physician, and family on functional limitations and progress towards goals.
  • Prepare a written discharge summary when physical therapy services are discontinued.
  • Instruct patient, family, and other hospice team members in certain phases of physical therapy.

Requirements

  • Current Physical Therapy licensure in state of practice
  • Current CPR certification
  • Current driver's license and vehicle insurance
  • Access to a dependable vehicle or public transportation
  • Ability to function in any home situation regardless of client's age, race, creed, color, sex, disability, or financial condition
