About This Role

This role involves providing clinical pharmacy services in an inpatient setting, optimizing drug therapy, and ensuring safe and cost-effective medication management for patients of all ages. The Pharmacist will rotate through central dispensing and decentralized clinical areas, contributing to patient care teams and quality improvement initiatives.

Responsibilities

  • Perform clinical services to optimize drug therapy and document clinical interventions
  • Provide medication therapy management using medication-related protocols including pharmacokinetics, parenteral nutrition, serum creatinine and other therapeutic drug monitoring
  • Advise physicians on use of formulary medications and suggest alternative therapeutic options
  • Provide accurate, clinically relevant drug information to physicians, nurses, and allied health professionals
  • Participate in code arrest response by providing information and assuring availability of medications
  • Identify, prevent, and investigate potential and real adverse drug reactions and medication errors
  • Dispense and label medication orders, including oral medications and IV solutions
  • Process patient charges and credits
  • Exercise applicable legal and professional controls on controlled substances
  • Supervise and provide oversight of supportive personnel

Requirements

  • Bachelor's Degree in pharmacy from an accredited college of pharmacy
  • Arizona State Board of Pharmacy license that is active and in good standing
  • BLS certification
  • ACLS certification

Qualifications

  • Bachelor's Degree in pharmacy from an accredited college of pharmacy
  • 3 months to 1 year experience preferred

Nice to Have

  • Doctor of Pharmacy degree from an accredited college of pharmacy
  • 3 months to 1 year experience
  • PALS certification for positions working in Emergency and Pediatrics departments
