Per Diem Nurse Practitioner / NP (Hospice)

About This Role

Deliver comprehensive hospice services under a collaborating physician's supervision, including patient evaluations, health assessments, diagnosis, and management of care plans.

Responsibilities

• Deliver comprehensive hospice services in accordance with the Nurse Practice Act and under a collaborating physician’s supervision
• Conduct patient evaluations, health assessments, diagnosis, and management of care plans, including prescribing medications and treatments
• Perform physical exams, face-to-face assessments, and hospice eligibility evaluations in consultation with the interdisciplinary team
• Collaborate with patients, families, and healthcare practitioners to coordinate care
• Utilize community resources to develop and direct services supporting patients and families at home and in the community

Requirements

• Graduate of an accredited School of Nursing and Nurse Practitioner Program
• Current RN and NP licensure in the state
• CPR certification
• Master’s degree
• At least one (1) year NP experience
• At least one (1) year hospice or palliative care experience
• Certification by an approved advanced practice nursing certifying body
• Strong assessment, communication, problem-solving, and nursing skills
• Able to meet physical demands of patient care
• Valid driver’s license
• Reliable insured vehicle
• Meet organization driving requirements

Qualifications

• Graduate of an accredited School of Nursing and Nurse Practitioner Program
• At least one (1) year NP experience and one (1) year hospice or palliative care experience

Nice to Have

• Specialty certification (e.g., CHPN, Pediatrics, Geriatrics)
