Deliver comprehensive hospice services under a collaborating physician's supervision, including patient evaluations, health assessments, diagnosis, and management of care plans.
Responsibilities
- Deliver comprehensive hospice services in accordance with the Nurse Practice Act and under a collaborating physician’s supervision
- Conduct patient evaluations, health assessments, diagnosis, and management of care plans, including prescribing medications and treatments
- Perform physical exams, face-to-face assessments, and hospice eligibility evaluations in consultation with the interdisciplinary team
- Collaborate with patients, families, and healthcare practitioners to coordinate care
- Utilize community resources to develop and direct services supporting patients and families at home and in the community
Requirements
- Graduate of an accredited School of Nursing and Nurse Practitioner Program
- Current RN and NP licensure in the state
- CPR certification
- Master’s degree
- At least one (1) year NP experience
- At least one (1) year hospice or palliative care experience
- Certification by an approved advanced practice nursing certifying body
- Strong assessment, communication, problem-solving, and nursing skills
- Able to meet physical demands of patient care
- Valid driver’s license
- Reliable insured vehicle
- Meet organization driving requirements
Qualifications
- Graduate of an accredited School of Nursing and Nurse Practitioner Program
- At least one (1) year NP experience and one (1) year hospice or palliative care experience

- Specialty certification (e.g., CHPN, Pediatrics, Geriatrics)
