Pediatric Licensed Practical Nurse (LPN)

About This Role

This role involves delivering and supervising patient care for infants, children, and adolescents primarily in their homes, utilizing nursing processes to assess, diagnose, plan, implement, and evaluate care. The LPN will work under the direction of clinical supervision staff to ensure quality care aligned with physician orders and regulatory standards.

Responsibilities

• Provide direct patient care to infants, children, and adolescents primarily in their home according to the active plan of care
• Provide services in accordance with a physician’s orders and regulations
• Perform comprehensive patient assessments
• Administer prescribed medications, treatments, and therapies
• Document timely, clear, concise, and accurate clinical notes
• Notify the RN Case Manager of any changes in the patient’s condition
• Provide and document education to patients and their families on diagnosis, treatment, and prevention
• Collaborate with other professional disciplines to ensure quality care
• Use appropriate infection control procedures and safety

Requirements

• Current State Licensed Practical Nursing (LPN) License
• Current CPR Certification
• Reliable transportation to office and patients’ homes

Qualifications

• One (1) Year Home Health Experience Preferred, One (1) Year Pediatric Experience Preferred
