Palliative Care RN

About This Role

Provide expert, individualized coordination of palliative care to patients and families, focusing on comprehensive assessment, symptom management, and connecting patients with community resources for improved health and well-being.

Responsibilities

• Review referrals for palliative care patients
• Provide expert, individualized coordination of care to patients and families
• Complete a comprehensive physical and psychosocial assessment of the patient and family
• Ensure appropriate referrals to specialists and support adherence to treatment plan
• Assist patients in accessing appropriate services and resources in the community
• Assist Nurse Practitioners in day to day program tasks and tracking palliative referrals
• Alleviate pain, fatigue, nausea, and other distressing symptoms
• Accurately document patient assessments, interventions, responses, needs, goals, and teaching in the EHR

Requirements

• Current RN licensure within state of branch location
• Three years of clinical nursing experience
• Successfully completed nursing education from an approved school of nursing
• Ability to independently organize work, communicate patient needs, and provide patient education
• Effective communication skills (oral and written)
• CPR certification
• Minimum of twelve hours of continuing education in a twelve-month period
• Valid driver's license

Qualifications

• Successfully completed nursing education from an approved school of nursing
• Three years of clinical nursing experience, preferably in Home Care, Hospice or Skilled Nursing facility setting
