Provide expert, individualized coordination of palliative care to patients and families, focusing on comprehensive assessment, symptom management, and connecting patients with community resources for improved health and well-being.
Responsibilities
- Review referrals for palliative care patients
- Provide expert, individualized coordination of care to patients and families
- Complete a comprehensive physical and psychosocial assessment of the patient and family
- Ensure appropriate referrals to specialists and support adherence to treatment plan
- Assist patients in accessing appropriate services and resources in the community
- Assist Nurse Practitioners in day to day program tasks and tracking palliative referrals
- Alleviate pain, fatigue, nausea, and other distressing symptoms
- Accurately document patient assessments, interventions, responses, needs, goals, and teaching in the EHR
Requirements
- Current RN licensure within state of branch location
- Three years of clinical nursing experience
- Successfully completed nursing education from an approved school of nursing
- Ability to independently organize work, communicate patient needs, and provide patient education
- Effective communication skills (oral and written)
- CPR certification
- Minimum of twelve hours of continuing education in a twelve-month period
- Valid driver's license
Qualifications
- Successfully completed nursing education from an approved school of nursing
- Three years of clinical nursing experience, preferably in Home Care, Hospice or Skilled Nursing facility setting
Nice to Have
- Experience in Home Care, Hospice or Skilled Nursing facility setting
Certifications
CPR certification
