Join a palliative care team to provide comprehensive care for Kaiser Permanente Georgia members with life-limiting illnesses, focusing primarily on outpatient assessments, treatment, and recommendations while collaborating with an interdisciplinary team.
Responsibilities
- Conduct office, telephone, video, and inpatient visits, assessing and treating members referred to Palliative Care
- Address clinical issues and develop medical aspects of care plans, including recommendations for observations, lab tests, medications, referrals, and prescriptions
- Collaborate with the Palliative Care Team, PCPs, Oncologists, Cardiologists, and other involved physicians to optimize member's overall health and functional status
- Serve as a resource for other clinicians, nurses, clinical pharmacy specialists, social workers, and chaplains to enhance care
- Participate in Palliative Care-related meetings, reviewing team goals, activities, and quality metrics to improve care
- Participate in community presentations and continuing medical education activities regarding Palliative Care to enhance awareness and knowledge
Requirements
- Board certified in internal medicine or family medicine or other relevant certifications
- Board certified in palliative care medicine
- One-year post-residency experience in caring for palliative care patients in an outpatient and/or facility setting
- Demonstrated ability to effectively communicate with patients with potentially life-limiting illness, their family members/caregivers, and other health professionals
- Ability and willingness to travel to various locations for patient care
Qualifications
- One-year post-residency experience in caring for palliative care patients in an outpatient and/or facility setting
Nice to Have
- Two years post-residency experience in providing palliative care services to patients in the outpatient and/or hospital/facility setting
- Experience working as part of an interdisciplinary team
Certifications
Board certified in internal medicine or family medicine or other relevant certifications
