The Outreach and Engagement Specialist performs vital outreach and engagement functions for eligible adults and children/families, helping them reconnect with treatment services and achieve their behavioral health goals. This role involves direct interaction primarily through phone and home visits, building trust, and coordinating with clinical teams.
Responsibilities
- Complete outreach activities with individuals who have potentially disengaged from their treatment services
- Assist clients in reconnecting with their treatment providers and objectives
- Engage with clients, families, and involved parties through phone interactions and home visits
- Work with clients and families to build trust, encourage hope, and ensure ongoing engagement in behavioral health services
- Coordinate and communicate clinically relevant information with the person's managing clinician
- Adhere to Clinic and AHCCCS-established outreach/re-engagement guidelines
- Complete necessary documentation promptly to support allowable billing of outreach activities
- Maintain client records in compliance with Mohave Mental Health Clinic, Inc., ADHS, RBHA/health plans, AHCCCS and Joint Commission standards
- Provide customer service to internal and external stakeholders
- Participate in the Performance Improvement processes in the Clinic
- Effectively de-escalate clients using techniques as trained in approved crisis intervention models
Requirements
- High school diploma or equivalency or higher
- Valid AZ driver's license and proof of insurance
- Arizona fingerprint clearance 21 yrs of age or older
- Certified in CPR and First Aid
- Strong written and verbal communication skills
- Strong interpersonal and customer service skills
- Problem-solving skills including conflict resolution skills
- Technical skills including windows computer program, copier, fax
- Resource management and organizational skills
Qualifications
- High school diploma or equivalency or higher
Nice to Have
- Knowledge of community resources available, including benefits and entitlement process, as well as an understanding of managed care
- Understanding of client/family-centered care model
