The Occupational Therapist provides home health services to patients to restore function, improve mobility, relieve pain, and prevent physical disabilities within the High Point, Kernersville, and Winston-Salem service areas. This role involves managing therapy plans, assessing patient problems, and collaborating with an interdisciplinary team.
Responsibilities
- Provide services to restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities of patients suffering from injuries and disease
- Manage the therapy Plan of Treatment for a case load of Integrated Health patients
- Collaborate with other members of the interdisciplinary team (IDT)
- Assess and identify patient problems
- Establish therapy goals and interventions
- Work with physician to establish a treatment plan using exercises, stretching, hands-on therapy, and equipment
- Establish and maintain therapeutic relationship with patient, including providing community support for resources and education
- Evaluate and treat hospice patients as needed
- Document the goals and outcomes of care
Requirements
- Degree from an accredited Occupational Therapy Program
- Current and unrestricted OT licensure
- Current CPR certification
- Valid state-issued driver's license
- Automobile liability insurance at limits required by agency
- Own transportation
Qualifications
- Degree from an accredited Occupational Therapy Program
- A minimum of six months of occupational therapy experience preferred
