About This Role

The Occupational Therapist provides home health services to patients to restore function, improve mobility, relieve pain, and prevent physical disabilities within the High Point, Kernersville, and Winston-Salem service areas. This role involves managing therapy plans, assessing patient problems, and collaborating with an interdisciplinary team.

Responsibilities

  • Provide services to restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities of patients suffering from injuries and disease
  • Manage the therapy Plan of Treatment for a case load of Integrated Health patients
  • Collaborate with other members of the interdisciplinary team (IDT)
  • Assess and identify patient problems
  • Establish therapy goals and interventions
  • Work with physician to establish a treatment plan using exercises, stretching, hands-on therapy, and equipment
  • Establish and maintain therapeutic relationship with patient, including providing community support for resources and education
  • Evaluate and treat hospice patients as needed
  • Document the goals and outcomes of care

Requirements

  • Degree from an accredited Occupational Therapy Program
  • Current and unrestricted OT licensure
  • Current CPR certification
  • Valid state-issued driver's license
  • Automobile liability insurance at limits required by agency
  • Own transportation

Qualifications

  • Degree from an accredited Occupational Therapy Program
  • A minimum of six months of occupational therapy experience preferred
