About This Role

Conduct nutrition screening, assessment, and counseling for diverse populations with various nutrition-related diseases. Implement nutrition care plans and assist in the development and implementation of educational materials and programs.

Responsibilities

  • Conducts nutrition screening, assessment and counseling, and implements nutrition care plans for low and high risk patients with diabetes, pre-diabetes, gestational diabetes, and other high risk pregnancies, hypertension, obesity, hyperlipidemia
  • Provides individual nutrition and health counseling for those with overweight, diabetes, hypertension and dyslipidemia
  • Maintains documentation by charting SOAP notes in the progress notes
  • May develop and participation group nutrition education programs and medical group visits
  • Collects and reviews material for education such as pamphlets, audio-visual aids, books and handouts
  • Assists in data entry and retrieval and other quality improvement efforts for the department
  • Promotes site based employee wellness efforts
  • May provide in-service nutrition/health education to staff and community partners
  • Participates in meetings related to nutrition, clinical health education and wellness as required/assigned

Requirements

  • RD or RD eligible
  • 1 year experience working in a community health care setting
  • Current BLS (Basic Life Support) Certificate
  • Knowledge of contemporary principles and practices of nutrition, nutritional value of foods, interpretation and implementation of special diets, and meal planning and preparation that is culturally appropriate
  • Knowledge and experience with Motivational Interviewing, Stages of Change, Division of Responsibilities
  • Interpretation of anthropometric, biochemical, clinical, and dietary information, and determination of appropriate dietary advice that is culturally appropriate
  • Knowledge of aspects of storing and/or preparing foods properly; interpretation and implementation of special diets; scopes functions and responsibilities of local health departments
  • Ability to develop, implement, and evaluate nutrition education activities and coordinate them with other departmental programs
  • Ability to work across cultures and demonstrate support of diversity, equity and inclusion
  • Bilingual English/Spanish

Qualifications

  • Completion of Dietetic and Food Administration Program from an accredited university
  • 1+ year experience working in a community health care setting
