Nurse Practitioner PRN

About This Role

Conduct comprehensive health assessments for adult and geriatric members in their homes or Skilled Nursing Facilities, reviewing medical history, medications, and social environment to improve health outcomes and identify chronic conditions.

Responsibilities

• Conduct adult and geriatric assessments including medical history, diagnosis, treatment, and health education
• Administer and order point-of-care and lab-based testing as needed
• Work with physicians, case managers, social workers, family members, and ancillary medical personnel
• Collaborate with Primary Care Physician (PCP) on patient education and follow-up
• Provide services at the member’s place of residence (homes or Skilled Nursing Facilities)

Requirements

• Master’s Degree or commensurate experience and satisfactory completion of NP licensure
• Current NP licensure in state of practice to include prescription authority
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
• Current BLS, ACLS or CPR certification
• Valid state driver's license for the state of residence
• Automobile insurance coverage for the state of residence with specific bodily injury, property damage, collision, and comprehensive deductibles
• Strong computer skills
• Familiarity with employee health/medical record software
• Excellent verbal and written communication skills

Qualifications

• Master’s Degree or commensurate experience and satisfactory completion of NP licensure
• 1 year of NP experience preferred, new grads encouraged to apply
