Nurse Practitioner - PRN

About This Role

Conduct comprehensive health assessments for older adults and at-risk individuals in their homes or skilled nursing facilities, focusing on medical history, diagnosis, treatment, and health education to improve health outcomes and close care gaps.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, health education, physician and case management referrals, follow-up, and clear documentation
• Administer and order point of care and lab-based testing as needed
• Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate
• Collaborate with Primary Care Physician (PCP) on patient education and follow up
• Provide services at the member’s place of residence, including homes or Skilled Nursing Facilities

Requirements

• Master’s Degree or commensurate experience and satisfactory completion of NP licensure
• Current NP licensure in state of practice to include prescription authority or ability to obtain
• AANP, AACN, or ANCC board certification in specific specialties (Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care)
• Current BLS, ACLS or CPR certification
• Valid state driver's license for the state of residence
• Automobile insurance coverage meeting specified bodily injury and property damage limits
• Strong computer skills and familiarity with employee health/medical record software
• Excellent verbal and written communication skills

Qualifications

• Master’s Degree or commensurate experience and satisfactory completion of NP licensure
• 1 year of NP experience preferred, new grads encouraged to apply
