Conduct comprehensive health assessments for adult and geriatric members in their homes or skilled nursing facilities, focusing on medical history, diagnosis, treatment, and health education. This role aims to improve quality of care and identify chronic conditions.
Responsibilities
- Conduct adult/geriatric assessments including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols
- Administer and order point of care and lab-based testing as needed
- Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate
- Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary
- Provide services at the member’s place of residence, which can be in their homes or Skilled Nursing Facilities
Requirements
- Master’s Degree or commensurate experience and satisfactory completion of NP licensure
- Current NP licensure in state of practice and prescriptive authority
- AANP, AACN, or ANCC board certification
- Current BLS, ACLS or CPR certification
- Valid state driver's license for the state of residence
- Automobile insurance coverage meeting specified limits
- Strong computer skills and familiarity with employee health/medical record software
- Excellent verbal and written communication skills
Qualifications
- Master’s Degree or commensurate experience and satisfactory completion of NP licensure
- 1 year of NP experience preferred, new grads encouraged to apply!
