Nurse Practitioner - PRN

About This Role

Conduct adult and geriatric health assessments, review medical history, and provide health education to improve quality of care and identify chronic conditions. Work collaboratively with medical personnel and provide services at the member's place of residence.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, and treatment
• Provide health education and make physician/case management referrals
• Administer and order point of care and lab-based testing as needed
• Work with physicians, case managers, social workers, family members, and ancillary medical personnel
• Collaborate with Primary Care Physician (PCP) on patient education and follow up
• Provide services at the member’s place of residence (homes or Skilled Nursing Facilities)

Requirements

• Master’s Degree or commensurate experience and satisfactory completion of NP licensure
• Current NP licensure in state of practice (including prescription authority or ability to obtain)
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
• Current BLS, ACLS or CPR certification
• Valid state driver's license for the state of residence
• Automobile insurance coverage meeting specified limits
• Strong computer skills and familiarity with employee health/medical record software
• Excellent verbal and written communication skills

Qualifications

• Master’s Degree or commensurate experience and satisfactory completion of NP licensure
• 1 year of NP experience preferred, new grads encouraged to apply
