About This Role

Serve as a primary care provider to long and short-term residents at a skilled nursing facility, collaborating with medical staff to deliver high standards of care and building relationships with residents and families.

Responsibilities

• Serve as primary care provider to long and short term residents
• Assess patient’s health by visiting patients; performing physical examinations; obtaining, updating, and studying medical histories
• Collaborate with medical attending/physician and nursing home staff to provide the highest standard of care attainable
• Build relationships with residents and families, educating and formulating a plan of evidence-based care congruent with patients’ needs and desires
• Comply with federal, state, and local legal and professional requirements
• Maintain professional and technical knowledge by attending educational workshops and reviewing professional publications
• Maintain safe and clean working environment by complying with procedures, rules, and regulations
• Review and ensure charting, documenting, and administration of care is up to date and performed in a timely and efficient manner
