Nurse Practitioner

About This Role

This role involves providing personalized, compassionate medical care to patients, primarily those with chronic illness or kidney disease, in their homes and virtually. The Nurse Practitioner or Physician Assistant will help improve patients' quality of life, slow disease progression, and coordinate care with integrated Care Teams.

Responsibilities

• Conduct assessments, including comprehensive annual wellness exams on patients both in their home and virtually
• Counsel and educate patients and families about available benefits and programs
• Document appropriate chief complaint, all applicable diagnoses, medical history, review of systems, examinations, medications, allergies, assessment, and plan
• Coordinate care with primary care providers, specialists, and appropriate ancillary services
• Complete all documentation and paperwork in a timely manner
• Maintain quality of care standards as defined by the practice
• Deliver evidence-based, timely care to reduce avoidable hospitalizations and maximize quality of life
• Prescribe medications, order tests, and collaborate with the patient’s physician
• Facilitate closing gaps in care by educating patients about preventive monitoring and scheduling diagnostic testing
• Participate in integrated care team meetings

Requirements

• Ability to work without direct supervision and practice autonomously
• Access to transportation and car insurance
• Proficient with medical instruments and equipment
• Knowledge of computer-based data management programs and information systems, medical records, and point-of-interview technology
• Ability to communicate effectively in verbal and written form
• Sound understanding of all federal and state regulations including HIPAA and OSHA

Qualifications

• 2 or more years of direct patient care required

Nice to Have

• Managed Care/IPA/Health Plan experience
• Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home
