The Nurse Practitioner will provide comprehensive in-home medical care to older adults and individuals with complex health needs, diagnosing and treating conditions, prescribing medications, and developing care plans. This role focuses on enhancing patient well-being and quality of life in their homes.
Responsibilities
- Provide direct patient care, including diagnosing and treating health conditions
- Prescribe medications and develop care plans
- Conduct patient assessments
- Collaborate with interdisciplinary care teams
- Maintain up-to-date medical records
- Provide compassionate care while enhancing patient well-being and quality of life
Requirements
- Strong clinical skills in patient evaluation, diagnosis, and treatment
- Proficiency in medical charting, record-keeping, and electronic health systems
- Board-certified Nurse Practitioner
- Valid VA license or eligibility for licensure
- CPR/BLS certification
- Ability to work on-site and travel to patients' homes
Qualifications
- Minimum of 2 years of experience in primary care or a related field preferred
Nice to Have
- Experience with palliative care, chronic disease management, and patient education
- Effective communication and collaboration with multidisciplinary care teams
