This Nurse Practitioner role provides physician-level oversight for homebound patients recently discharged from hospitals, focusing on stabilizing their health and preventing readmissions. The position is critical for improving post-acute care and supporting patients during crucial transition periods.
Responsibilities
- Serve as the clinical anchor for homebound patients after discharge from the hospital
- Provide structured, physician-level oversight to stabilize patients to prevent avoidable readmissions
- Create consistency in patient care and follow-up
- Return patients to appropriate levels of care
- Capture meaningful insights that lead to meaningful interventions in patient care
- Conduct assessments and treat patients
- Conduct virtual consultations and develop individualized recovery plans
- Collaborate with other healthcare professionals and partners
- Monitor patient progress and adjust care plans as necessary
- Deliver compassionate support to patients and their families
Requirements
- Proven clinical expertise, including conducting assessments, and treating patients
- Experience with high-risk or homebound patients
- Strong communication and interpersonal skills for effective patient and team interactions
- Proficient in utilizing telehealth or virtual care technologies
- Ability to travel locally
- Compassionate, adaptable, and committed to delivering patient-centered care
