The NRHA Navigator will support residents from under-resourced communities towards self-sufficiency by providing comprehensive case management, conducting in-depth intake assessments, and connecting individuals with essential community resources.
Responsibilities
- Travel to meet with clients at their home, community partner offices, or satellite offices to ensure access to services
- Maintain detailed case records, complete required program documentation, and input timely case data into Apricot database
- Collaborate with Lead Navigators and the NRHA team to provide support to participants/families on their journey towards health, wellness, and self-sufficiency
- Schedule and complete in-depth intake assessments with all new referrals
- Support Outreach and Engagement activities during evenings and weekends as needed
- Build trusting relationships with participants and their families, providing general support and encouragement
- Provide ongoing follow-up, basic motivational interviewing, and goal setting with participants/families
- Help participants set personal health related goals and attend appointments
- Provide referrals for services to community agencies as appropriate
- Record participant care management information in the database and other software no later than 24 hours after participant contact
- Maintain and manage a caseload of at least 25 participants
Requirements
- Customer focused, data-driven, organized, and results oriented
- Ability to work on a flexible schedule that may include evenings and/or weekends
- Own vehicle, acceptable driving record, and ability to travel independently to locations across South Hampton Roads
- Experience in human services programs
- Successful completion of a Community Health Worker formal training program within 90 days of employment
- Knowledge of best practices in human services and of systems addressing the needs of Housing Choice Voucher Program participants and the ALICE population
- Ability to maintain respect for the confidentiality of information divulged by or on behalf of participants
- Ability to build credible relationships with community members and partners
- Ability to collaborate and work in a team environment as well as work independently and make sound decisions
- Able to multi-task, meet deadlines, keep schedules, manage relationships, and perform in a high-paced environment
- Excellent written and oral communication skills
- Experience with MS Outlook, Word, and Excel, PowerPoint
- Strong detail orientation and self-organized

- 1-2 years of experience in human services programs
