The Navigator I enhances client success by identifying and reducing barriers, connecting individuals with essential services and resources. This role involves outreach, engagement, and case management for clients needing additional support.
Responsibilities
- Enhance the likelihood of client success
- Identify and reduce barriers for clients
- Connect clients with needed services and resources
- Conduct outreach and engagement activities
- Provide case management and ongoing wrap around supportive services
- Map out appropriate next steps for individuals accessing various services
- Document and track required patient interactions according to regulatory guidelines
Requirements
- Highschool diploma or GED
- Minimum of six months of recovery from substance use and/or mental health disorders
Qualifications
- Highschool diploma or GED is required. Associates Degree (or higher) in a field related to Behavioral Health is preferred but not required.
- Minimum of six months of recovery from substance use and/or mental health disorders required. 1-3 years of full-time Health Care related work experience and substance abuse/mental health recovery-oriented service preferred.
Nice to Have
- Associates Degree (or higher) in a field related to Behavioral Health
- 1-3 years of full-time Health Care related work experience with behavioral and medical crisis situations
- 1-3 years of substance abuse/mental health recovery-oriented service
