The Navigator will assist young adults transitioning from foster care to independent living in Maricopa County, connecting them with resources and supporting life skills development.
Responsibilities
- Maintain contact with young adults, collateral persons and agencies to assist with life skills and goal accomplishment
- Collaborate with young adult clients to develop goals and service plans using a strength-based approach
- Monitor progress and provide resources and tools as needed
- Participate in group training and educational meetings
- Provide individual support and services focusing on life skills development, including tutoring and transportation
- Develop and facilitate trainings and workshops on independent living skills
- Assist young adults in developing and maintaining contracts with the Department of Child Safety if applicable
- Assist young adults in developing and accessing scholarships and grants for education and training
- Maintain accurate client records, completing case notes and supplemental documentation
- Complete and provide required monthly, quarterly and annual data reports
Requirements
- Must be 21 years of age
- Valid Arizona driver's license and insurable under agency's automobile policy
- Ability to provide own transportation
- Ability to obtain DPS fingerprint clearance
- Meet training and agency compliance requirements
Qualifications
- Bachelor's degree with 2 years of related experience
- 2 years of related experience in child welfare or behavioral health fields working with young people ages 14-21 who are involved in systems of care (foster care, juvenile justice, runaway and home homeless youth programs)
