Mental Health Practitioner

About This Role

Provide direct mental health services to adults in the community, at home, or in the office. Work collaboratively with a team to facilitate individual and group services, teach independent living skills, and coordinate referrals.

Responsibilities

• Maintain a caseload of clients
• Develop and work with all members of the treatment plan team
• Facilitate services to clients individually and in a group setting
• Coach, teach, educate, and model independent living skills to clients
• Coordinate and make further referrals for clients as needed
• Seek treatment consultation from Clinical Supervisor
• Accurately complete all required documentation and notes for each client in a timely manner

Requirements

• Bachelor’s degree in Social Work, Psychology or a related human services field
• One year of experience providing services to individuals with a mental illness or substance use disorder
• Qualify as a Mental Health Practitioner in the State of MN
• Valid driver's license and proof of auto insurance
