Provide direct mental health services to adults in the community, at home, or in the office. Work collaboratively with a team to facilitate individual and group services, teach independent living skills, and coordinate referrals.
Responsibilities
- Maintain a caseload of clients
- Develop and work with all members of the treatment plan team
- Facilitate services to clients individually and in a group setting
- Coach, teach, educate, and model independent living skills to clients
- Coordinate and make further referrals for clients as needed
- Seek treatment consultation from Clinical Supervisor
- Accurately complete all required documentation and notes for each client in a timely manner
Requirements
- Bachelor’s degree in Social Work, Psychology or a related human services field
- One year of experience providing services to individuals with a mental illness or substance use disorder
- Qualify as a Mental Health Practitioner in the State of MN
- Valid driver's license and proof of auto insurance
