This role involves partnering with pharmacies and physicians to implement and grow Remote Patient Monitoring (RPM) programs, focusing on educating providers, integrating workflows, and supporting senior populations to enhance proactive, data-driven care and independence at home.

Responsibilities

Educate providers on RPM program value, workflow, and patient impact
Support practices and pharmacies in implementing remote care models
Strengthen patient engagement through proactive monitoring strategies
Build long-term provider partnerships centered on service, outcomes, and support

Requirements

5+ years in healthcare, medical, pharma, DME, or related provider-facing roles
Experience working with physicians, pharmacies, or care teams
Strong relationship-building and program implementation mindset
Self-directed, entrepreneurial, and growth-oriented
