The Medical Case Manager is responsible for providing comprehensive medical case management and treatment adherence support for residents. This role involves advocating for residents to access essential medical and support services, ensuring coordination of care, and promoting client self-sufficiency.
Responsibilities
- Ensure all contacts and documentation are completed in a timely manner and up to date.
- Obtain referrals for medical care and communicate resident’s needs with the Supportive Housing Team.
- Perform home visits to residents and attend medical appointments.
- Educate clients and the Supportive Housing Team on treatment adherence, health and human service resources, assist in obtaining services, and follow-up on service delivery.
- Interact with service providers in a professional manner while advocating for residents.
- Appropriately intervene in situations requiring immediate attention to ensure safety of residents.
- Maintain knowledge of resident’s medical insurance and ensure proper billing information is provided at each medical visit.
- Assist with selecting a Managed Long Term Care (MLTC) plan and enrolling.
- Coordinate referrals to Home Health Care and assisted living facilities.
- Use own vehicle to travel to meet residents for home visits and appointments.
Requirements
- Bachelor's degree in health, human services, or nursing OR Associate's degree (or 60 college credit hours) in health, human services, or education OR LPN OR CASAC
- 1 year of qualifying experience (with Bachelor's) or 2 years of qualifying experience (with Associate's/LPN/CASAC)
- Valid driver’s license
- Safe driving record
- Valid auto insurance
- Access to a vehicle
Qualifications
- Bachelor's degree in health, human services or nursing preferred OR Associate's degree (OR 60 credit hours of college study) in health, human services, or education, OR LPN, OR CASAC.
- 1 year of qualifying experience with a Bachelor's degree, or 2 years of qualifying experience with an Associate's degree/LPN/CASAC. Qualifying experience includes case management or casework with persons with HIV or AIDS, and/or persons with history of mental illness, homelessness, or chemical dependence, or other populations in need.
