Medical Assistant - Clinical Navigator

About This Role

This role involves delivering remote patient care and health coaching to individuals managing chronic conditions like type 2 diabetes and hypertension. The Medical Assistant will support patients by reinforcing care plans and providing education, working in partnership with a clinical care team.

Responsibilities

• Support delivery of healthcare to patients by performing activities and procedures under the direction of the Nurse Practitioner and clinical policies.
• Provide one-on-one coaching and support to patients managing chronic conditions.
• Conduct comprehensive assessments of patients' health status, lifestyle behaviors, nutritional habits, and readiness to change.
• Help patients execute personalized care plans and goals, focusing on behavior modification, nutrition, physical activity, and self-management strategies.
• Monitor patients' progress, adherence to treatment plans, and health outcomes through regular check-ins and remote monitoring tools.
• Educate patients on disease management, medication adherence, symptom recognition, and prevention strategies.

Requirements

• Active Certification as a Medical Assistant from an accredited association.
• 5+ years of experience as a Medical Assistant.
• Consistently stable WiFi with strong upload and download speeds for remote work.
• Highly skilled in behavioral-based coaching.
• Excellent clinical acumen and competency in patient assessment.
• Excels in patient support and delivers a high level of service.
• Consistent track record of attendance and adherence to work schedules.
• Exceptional written, verbal, and interpersonal communication skills.
• Reliability in meeting deadlines and fulfilling job duties.
• Works effectively with minimal supervision and is consistently punctual.
• Prior experience working in a high-growth, fast-paced startup environment while maintaining high clinical standards.
• Experience in patient education for individuals managing chronic conditions.
• Tech-savvy and capable of working with multiple systems.

Qualifications

• 5+ years of experience as a Medical Assistant.
