About This Role

This role involves joining a rapidly growing practice to manage and coordinate patient care following admissions, procedures, and discharges. The goal is to ensure safe transitions of care, support early identification of complications, and facilitate follow-up appointments.

Responsibilities

  • Receive and review procedural notes for cardiology patients
  • Contact patients post-discharge to conduct medication reconciliation, verify prescriptions, and ensure understanding of discharge instructions
  • Schedule and coordinate follow-up visits with cardiologists, advanced practice providers, and diagnostic services
  • Collaborate with the Intensive Cardiac Rehabilitation team to facilitate patient enrollment or referral
  • Support adherence to care plans by addressing barriers such as transportation or medication access
  • Serve as a clinical resource to patients and families, offering guidance and triage
  • Document patient interactions thoroughly within the EMR and escalate issues as needed
  • Assist with remote patient monitoring programs
  • Collaborate with care team members and hospital transition-of-care teams to ensure continuity
  • Act as a liaison between the cardiology practice and external providers

Requirements

  • Certified Medical Assistant
  • Current BLS certification
  • Minimum 2 years of clinical cardiology experience
  • Minimum 2 years of transitional care management and/or care coordination of cardiology patients
  • Knowledge of cardiovascular disease, diagnoses, and medications
  • Excellent communication skills
  • Ability to interpret clinical documentation and cardiac terminology
  • Strong organizational skills with attention to detail
  • Proficiency with Athena EMR systems and care-management platforms
  • Compassionate, patient-centered mindset

Qualifications

  • Minimum 2 years of clinical cardiology experience and 2 years of transitional care management and/or care coordination of cardiology patients
