Lead and supervise interdisciplinary hospice care teams, ensuring the delivery of high-quality, person-centered care to patients and families. Oversee care planning, staff development, compliance, and team collaboration within a compassionate hospice environment.
Responsibilities
- Provide direct supervision, coaching, and delegation to assigned interdisciplinary team members
- Ensure clinical care adheres to individualized plans, hospice regulations, organizational policies, and professional standards
- Coordinate and facilitate weekly Interdisciplinary Group (IDG) meetings for patient-facing care teams
- Review clinical documentation for timeliness, completeness, and regulatory compliance
- Support patient care transitions and collaborate on staffing and caseload distribution
- Provide real-time coaching for symptom management, family dynamics, ethical concerns, or complex care needs
- Participate in hiring, orientation, training, and evaluation of clinical staff
- Address quality, safety, and service concerns and implement corrective actions
- Assist in scheduling and coverage planning to meet patient needs, including after-hours or weekend situations
- Participate in Quality Assessment and Performance Improvement (QAPI) and compliance activities
- Model professionalism, accountability, and compassionate care
- Support documentation audits, surveys, or accreditation processes
Requirements
- Current RN license
- Minimum of three (3) years clinical experience in hospice, palliative care, or related setting
- Current CPR certification
- Strong clinical judgment, communication, and organizational skills
- Proficient in electronic medical record systems and Microsoft Office
- Ability to work in home and facility settings with reliable transportation
- Demonstrated empathy, sensitivity, and ability to support diverse patients and families
Qualifications
- Bachelor's degree in nursing or related field preferred.
- Minimum of three (3) years clinical experience in hospice, palliative care, or related setting; minimum of one (1) year leadership or supervisory experience preferred.
