About This Role

The LPN Team Leader provides skilled nursing care to residents while leading the neighborhood team. This role promotes Person-Centered Care, supervises daily resident care, and supports the professional growth of staff. Work schedule is every Friday, Saturday, and Sunday from 10:30 PM to 7:00 AM.

Responsibilities

  • Observe and report changes in resident condition to the Nurse Supervisor or physician/physician extender
  • Communicate with Supervisor, ADON, DON, NHA, care providers, and family/POA as needed
  • Use evidence-based practices to ensure safe, high-quality care delivery
  • Assist in managing personnel through performance evaluation, coaching, counseling, education, and corrective action
  • Complete all required documentation accurately and in a timely manner
  • Participate in resident transitions of care, care conferences, and care plan reviews
  • Collaborate with Cedarwood staff to ensure optimal resident outcomes
  • Comply with Infection Control practices
  • Ensure hand-held communication equipment is maintained for effective call bell management and team communication
  • Attend and complete all required in-services, training, and team meetings

Requirements

  • Licensed Practical Nurse (LPN) in the Commonwealth of Pennsylvania
  • High school diploma or equivalent
  • BLS Healthcare Provider certification
  • Basic computer skills
  • Strong communication skills, both verbal and written
  • Ability to role model professional appearance and demeanor
  • Demonstrates respect, integrity, compassion, and excellence
  • Commitment to person-centered care initiatives
  • Conflict resolution skills and ability to coach and support staff
  • Flexible schedule and willingness to work extra hours as needed

Qualifications

  • High school diploma or equivalent

Nice to Have

  • Prior experience in geriatric nursing
