About This Role

HealthPro Heritage is seeking a Licensed Physical Therapist Assistant to provide direct patient care in a home health setting, assisting with treatment plans and educating patients in Norwich, CT and surrounding areas. This role offers the flexibility to build your own schedule.

Responsibilities

• Assist in implementing treatment plans designed by Physical Therapists, providing direct patient care including therapeutic exercises, manual therapy, and modality applications.
• Monitor and document patient progress, report observations to the Physical Therapist, and educate patients and families on treatment protocols, exercises, and self-care techniques.
• Accurately document patient treatments, progress, and responses in compliance with facility policies and regulatory standards.
• Work closely with Physical Therapists, other healthcare professionals, and support staff to ensure coordinated patient care and effective treatment outcomes.

Requirements

• Associate’s degree in Physical Therapist Assisting from an accredited program
• Valid state licensure as a Physical Therapist Assistant, or license eligible
• Strong assessment, diagnostic, and therapeutic abilities with excellent communication and interpersonal skills
• Proficient in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems
• Proven ability to work collaboratively in a multidisciplinary team
