About This Role

This role involves providing in-person psychotherapy services to the geriatric population in skilled nursing facilities, focusing on assessing, diagnosing, and treating emotional and behavioral health conditions. The social worker will develop evidence-based treatment plans and collaborate with interdisciplinary teams to improve patient outcomes.

Responsibilities

  • Conduct individual and group psychotherapy sessions for patients in subacute or skilled nursing facilities
  • Assess, diagnose, and treat a wide range of emotional and behavioral health conditions including depression, anxiety, adjustment disorders, and cognitive decline
  • Develop and implement evidence-based treatment plans tailored to the needs of each patient
  • Collaborate with facility staff, interdisciplinary teams, and family members to support patient care
  • Maintain accurate and timely clinical documentation in accordance with regulatory standards using an electronic medical/health system (EHR)
  • Monitor patient progress and adjust treatment plans as needed

Requirements

  • Active and unrestricted license to practice as a Licensed Clinical Social Worker in the state of application
  • Effective oral and written communication in English
  • Basic proficiency with technology, including electronic health records (EHR)
  • Strong organizational and documentation skills, with attention to regulatory compliance
  • Ability to provide in-person services at assigned facilities
  • Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day

Nice to Have

  • Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting
  • History of treating Anxiety, Depression and Adjustment Disorders
  • Psych or Addictions experience
  • Excellent time management and organizational skills
